COMPLAINTS |

All persons have a right to file a complaint with the agency, regardless of whether or not they are receiving services from La Familia. If you would like to file a complaint, please download and complete the form below:

Once you complete the form, return it to the Compliance Officer at the above address. You will receive a written response within three working days of the date the agency receives the complaint.
If you are not satisfied with the response, a meeting will be arranged with the department or program director.
If you are not satisfied with the department or program director’s response, you may appeal, in writing, to the President and CEO.
The President and CEO will review the complaint, and will provide a written response within fourteen days from the date it was referred.
