La Familia's Care Management Services for Adults encompass a comprehensive range of care management and fiduciary services to older adults facing age-related challenges, to adults with developmental disabilities, and to families caring for adults.

Our multidisciplinary approach allows us to:

  • Provide comprehensive social, mental health, legal, and financial needs assessments
  • Prepare individualized care plans
  • Connect families with existing services
  • Coordinate consultation, education, and services

NEW: Two new online courses are now available to help you understand Guardianship and Conservatorship. They are Alternatives to Guardianship and Conservatorship and Powers and Duties of Guardians and Conservators.
